Anyone recognizes this selector-on-top PT?

I'm interested to know make/model, and if it was used in a specific amp model.

It served longtime as replacement in a modified JTM45, powering 3xECC83 + 2xEL34 with diode rectifier.
  • Selector on top: 110, 220, 245
  • Stack size: 3 3/4" x 3 1/8" x 1 7/8".
  • Resistance: Primary 6,1 Ohm (110), 17,1 Ohm (220), and 19,8 Ohm (245). Secondary, HT 43,5 + 40,0 Ohm.
  • Voltage: 3,15-0-3,15 and 335-0-335 VAC.
  • Wires: Two black primaries go in on top, HT red-green-red, heaters black-yellow-black (all solid), and one green wire without connection to any winding or PT shell (connected to chassis ground in amp).

Looks like a Radio Spares transformer from a '65-'66 JTM 45.
It doesn't have any 5V heater winding, for GZ34 or similar rectifier valve.

I thought about EZ81/EL84 type of amp, like Vox AC-10. Seems like all heaters are connected to same winding, and EZ81 heaters are not electrically connected to cathode (as I understand).
